School Leaver Program
Setting Up What Comes Next
Feeling unsure about life after school?
You’re not alone. For many families, this transition can feel overwhelming. The difference is having the right support early.
At Forge Disability Services, we don’t wait until school ends. We help you plan, trial, and build a clear pathway before your young person finishes.
Start Early. Get It Right.
The strongest transitions don’t happen at the end of Year 12. They start earlier.
From around Year 11, we work with families to:
- explore options without pressure
- build confidence in new environments
- introduce supports gradually
- align services with long-term goals
This means your young person finishes school with direction, not uncertainty.
We Work Around Your Young Person
This isn’t a handover. It’s a transition done properly.
We work alongside:
- families
- schools
- support coordinators and therapists
Together, we build a pathway that reflects your young person’s goals, strengths, and support needs.
This can include:
- attending school meetings
- transition planning
- trial days and gradual introductions
- consistent communication across everyone involved

Frequently Asked Questions
We have some answers for you. Please contact us if we haven’t answer all your questions.
What happens after leaving school?
After school, there are different pathways depending on what you want to work towards. This might include day programs or community participation. Our focus is on helping you build independence, routine, and connection.
How do I know if this program is right for me or my young person?
We take the time to understand goals, interests, and support needs. We also offer tours and trial days so you can see if the program feels like the right fit.
What will a typical day look like?
Each day includes a mix of life skills, community access, and social activities. This might include cooking, travel training, volunteering, fitness and building everyday independence.
Can supports be tailored to individual goals?
Yes. Supports are shaped around individual goals, with a focus on building confidence, independence, and real-life skills over time.
Is this funded through the NDIS?
Yes, supports are usually funded through your NDIS plan. We can help you understand how your funding can be used.
How do we get started?
The first step is to get in touch with our team. From there, we can organise a tour, talk through goals and support needs, and guide you through the next steps.
